Appointments

Appointments are available daily 8am-4:00pm Monday through Friday

When making an appointment inform the receptionist of the device prescribed to allow time for evaluation and measurements.

The receptionist will ask you questions for our patient information form or you may download it here, complete it and bring it with you or fax it ahead of time to our office.

*Coverage will be determined by the health plan with insurance companies that we are not contracted with.

However financial arrangements can be made to accommodate the patient in the event that services provided are not covered in full.
